Issue 3: Photo identification to vote (constitutional amendment)
Proposed Constitutional Amendment TO REQUIRE VOTERS TO PRESENT PHOTO IDENTIFICATION IN ORDER TO VOTE Proposed by Joint Resolution of the General Assembly To enact Section 5 of Article V of the Constitution of the State of Ohio A majority yes vote is required for the adoption of Section 5. This proposed amendment would: • Require voters to present an approved form of government-issued photo identification in order to vote. • Allow state law to provide for an alternative method of verifying a voter’s identity for a voter who has a sincere religious objection to being photographed and, therefore, does not possess an approved form of government-issued photo identification. • Require that voters who do not vote in person provide an approved form of government-issued photo identification, unless the law authorizes those persons to vote by providing a signature and at least one other specified unique identifier. • Define approved forms of government-issued photo identification as an unexpired Ohio driver license or state identification card; a United States passport or passport card; or a United States military identification card, Ohio National Guard identification card, or United States Department of Veterans Affairs identification card. If approved, the amendment shall take effect immediately. A “YES” vote means approval of the amendment. A “NO” vote means disapproval of the amendment. SHALL THE AMENDMENT BE APPROVED?
Fayette County Emergency Medical Service District levy (renewal)
A renewal of a tax for the benefit of Fayette County Emergency Medical Service District for the purpose of providing ambulance service, emergency medical service, or both, that the county auditor estimates will collect $1,482,933 annually, at a rate not exceeding 1.90 mills for each $1 of taxable value, which amounts to $49 for each $100,000 of the county auditor's market value, for 3 years, commencing in 2027, first due in calendar year 2028.
Washington Court House City School District income tax (renewal)
Shall an annual income tax of 1.00% on the earned income of individuals residing within the school district be imposed by the Washington Court House City School District to renew an income tax expiring at the end of 2027, for 7 years, beginning January 1, 2028, for the purpose of providing for current expenses.

Miami Trace Local School District operating levy (renewal)
Shall a fixed-sum levy renewing an existing levy be imposed by the Miami Trace Local School District for the purpose of paying the current operating expenses of the district in sum of $1,900,000 and a levy of taxes to be made outside of the ten mill limitation estimated by the county auditor to average 2.2 mills for each $1 of taxable value, which amounts to $77 for each $100,000 of the county auditor's market value, for a period of 5 years, commencing in 2027, first due in calendar year 2028? The county auditor had certified that the amount by which the carry-over balance in the District's general operating budget from the preceding fiscal year exceeds the District's general fund expenditures made in the preceding fiscal year is $0, which is 0% of those expenditures.

Union Township fire protection levy (renewal)
A renewal of a tax for the benefit of Union Township for the purpose of providing and maintaining fire apparatus, appliances, building or sites therefore, or sources of water supply and materials thereof, and the establishment and maintenance of lines of fire alarm, telegraph or the payment of permanent, part time or voluntary fireman or fire-fighting companies to operate the same or contract for said fire protection, that the county auditor estimates will collect $115,505 annually, at a rate not exceeding 1 mills for each $1 of taxable value, which amounts to $35 for each $100,000 of the county auditor's market value, for 5 years, commencing in 2027, first due in calendar year 2028.
