Issue 3: Photo identification to vote (constitutional amendment)
Proposed Constitutional Amendment TO REQUIRE VOTERS TO PRESENT PHOTO IDENTIFICATION IN ORDER TO VOTE Proposed by Joint Resolution of the General Assembly To enact Section 5 of Article V of the Constitution of the State of Ohio A majority yes vote is required for the adoption of Section 5. This proposed amendment would: • Require voters to present an approved form of government-issued photo identification in order to vote. • Allow state law to provide for an alternative method of verifying a voter’s identity for a voter who has a sincere religious objection to being photographed and, therefore, does not possess an approved form of government-issued photo identification. • Require that voters who do not vote in person provide an approved form of government-issued photo identification, unless the law authorizes those persons to vote by providing a signature and at least one other specified unique identifier. • Define approved forms of government-issued photo identification as an unexpired Ohio driver license or state identification card; a United States passport or passport card; or a United States military identification card, Ohio National Guard identification card, or United States Department of Veterans Affairs identification card. If approved, the amendment shall take effect immediately. A “YES” vote means approval of the amendment. A “NO” vote means disapproval of the amendment. SHALL THE AMENDMENT BE APPROVED?
Issue 5: Maumee City School District income tax
Shall an annual income tax of 1.25% on the earned income of individuals residing in the school district be imposed by the Maumee City School District, for a continuing period of time, beginning January 1, 2027, for the purpose of current expenses?

Issue 6: Springfield Local School District income tax
Shall an annual income tax of one-half per centum (0.50%) on the earned income of individuals residing in the school district be imposed by the Springfield Local School District, for a five (5) year period of time, beginning January 1, 2027, for the purpose of current expenses?

Issue 7: Sylvania City School District income tax with a property tax reduction
Shall an annual income tax of 1% on the earned income of individuals residing in the school district be imposed by the Sylvania City School District, for a continuing period of time, beginning January 1, 2027, for the purpose of current expenses, and shall the rate of an existing tax on property, currently levied for the purpose of current expenses at the rate of 4.9 mills, be REDUCED to 3.2 mills for each $1 of taxable value, which amounts to a reduction from $101 to $66 for each $100,000 of the county auditor's market value, that the county auditor estimates will collect $4,725,909 annually, the reduction continuing until any such time as the income tax is repealed?

Issue 8: Toledo Lucas County Public Library additional levy
An additional tax for the benefit of the Toledo Lucas County Public Library for the purpose of current expenses that the county auditor estimates will collect $34,261,921 annually, at a rate not exceeding 3 mills for each $1 of taxable value, which amounts to $105 for each $100,000 of the county auditor's market value, for 5 years, commencing in 2026, first due in calendar year 2027.
Issue 9: Toledo City School District additional levy
An additional tax for the benefit of Toledo City School District for the purpose of current expenses that the county auditor estimates will collect $22,817,500 annually, at a rate not exceeding 6.9 mills for each $1 of taxable value, which amounts to $242 for each $100,000 of the county auditor's market value, for 5 years, commencing in 2026, first due in calendar year 2027. The county auditor has certified that the amount by which the carry-over balance in the district's general operating budget from the preceding fiscal year exceeds the district's general fund expenditures made in the preceding fiscal year is $0, which is 0% of those expenditures.

Issue 13: Lucas County renewal levy for Imagination Station
A renewal of a tax for the benefit of the County of Lucas for the purpose of operation and maintenance of the Imagination Station, Toledo's non-profit science center, and facility to promote the sciences and STEM education, that the county auditor estimates will collect $1,339,989 annually, at a rate not exceeding 0.17 mill for each $1 of taxable value, which amounts to $3 for each $100,000 of the county auditor's market value, for five years, commencing in 2027, first due in calendar year 2028.
Issue 16: City of Toledo temporary 0.25% income tax
Shall Ordinance (Ordinance No. 301-26) providing for a new temporary one-quarter percent (.25%) City of Toledo levy on income for the period commencing on January 1, 2027, and ending December 31, 2030, which shall be in addition to the city of Toledo's other levies on income, for the purpose of allocating the levy proceeds to the General Fund to be dedicated to neighborhood safety expenditures, and to police and fire safety force operations, including criminal gang disruption, be passed?
Issue 17: City of Toledo charter amendment on City Council employees
Chapter I, Section 6 to be amended to remove outdated language and updating definition of employee to include members of the unclassified service. Chapter IV, Section 33 to be amended to allow City Council to determine the conditions of employment for its officers and employees, including matters such as hiring, discipline, and termination. Chapter VIII, Section 171 to be amended to designate that City Council officers and employees are members of the unclassified service. Shall the proposed amendments to the Charter of the City of Toledo, as set forth in Ordinance 302-26, be adopted?
Issue 18: City of Toledo charter amendments on election procedures
Chapter III, Section 14 to be amended to update language to be more aligned with state law, and in particular to remove the requirement that a candidate who has filed a nominating petition for a primary election must also file a written acceptance of nomination with election authorities. Chapter III, Section 17 to be amended to bring the criteria for designation as a write-in candidate in line with state election law by requiring any potential write-in candidate to declare their intent to run on the forms prescribed by the state. Chapter VI, Section 83 to be amended to increase the deadline for which an election on referendum or initiative petitions must be held within from 75 days to 90 days. Chapter VI, Section 92 to be amended to require any petitions filed to be consistent with the forms prescribed by the state. Shall the proposed amendments to the Charter of the City of Toledo, as set forth in Ordinance 304-26, be adopted?
Issue 19: City of Toledo charter amendment on how the Charter is amended
Shall Chapter I, Section 5 be amended to be consistent with the Ohio Constitution by requiring Council to approve proposed Charter amendments by an ordinance rather than a resolution, and to rewrite the Section to describe the requirements of the Charter amendment process more clearly, as set forth in Ordinance 305-26?
